PLEASE NOTE: You must call us at 1-800 DELILAH (1800-335-4524), and leave a message AND receive an email back from us regarding that message before filling out any of this paperwork! We can only hire people selected from those messages...
To get set up and begin working with 1800Delilah, please do these three things:
1. Learn about the position. Watch the orientation video (found here) and read over the website. If there's anything you don't understand about the position, just ask!
2. Answer the setup questions. You can find them here or link to them in the column to the right. Copy them into an email and email us your answers. Once I have your answers, I'll get you set up in our system. You'll receive an email from me confirming this once it's done.
3. Get your paperwork underway: Print and fill out the two forms
listed to the right (I/C agreement and W-9) along with your photo ID and social security card, and scan and email them all to admin@1800delilah.com... You can also fax everything, along with a copy
of your photo ID and social security card to 1 877 226 9309.
IMPORTANT: If scanning your documents to email them to us, please do not save them in picture format (.jpg, .tif, or .gif)! This creates an enormous file that often does not arrive or cannot be opened. When you are given save options, please choose .pdf format, as this creates a compact, easy-to-send and open file...
Contact us when this has been done and we will look for your email or fax.
We need the originals on file at our
Miami office as well, so put everything in an envelope and mail it to
the address to the right. Be sure to include the copy of your driver's
license or state-issued ID card, and a copy of your
social security card. Please make sure that copies
of DL or state-issued-ID card and SS card are clear and readable before
sending.
If you cannot fax or email your paperwork, simply mail the complete
package to the address to the right, and let us know that you have
done so. When all paperwork is received, you will be assigned an extension
number and will be able to begin working.
Address changes: Please keep us updated on any address
changes. This is especially important for you to receive your paychecks,
and your 1099 form for tax filings at the beginning of the year.
